Hospice At Home
Offering support at home in your time of greatest need
Our hospice at home service aims to do exactly that - bringing hospice care into the home environment.
It enables patients with a life limiting illness to be cared for at the end of their life in their own home and works alongside other community teams to provide basic nursing care and emotional support.
The service operates 7 days a week, 365 days a year, between the hours of 10pm and 7 am (night sits) and is dependent upon availability.
There is also a limited service available Monday to Friday 9am to 5pm which aims to provide a rapid response and interim care until other care agencies are able to provide the service.
