Offering support at home in your time of greatest need
Our hospice at home service aims to do exactly that - bring hospice care into the home environment, putting the patient and those who matter to them at the centre of care.
The wishes of our patients are central to the service we offer and it is clear that, in many cases, those wishes include the right to die peacefully at home.
Although this is not always possible, the extra help offered by the hospice at home service has made this achievable for many of our patients. Especially our night care service, when round-the-clock assistance is necessary, but not practical for a loved one to provide alone.
The hospice at home nurses carefully and sensitively assess the amount of care needed, working closely with the family and others involved in the patient’s care.
We offer to be there when you need us, for nursing care and assistance in the final days, and support for the patient’s loved ones.
